I understand that participation in Next Level Soccer Training involves physical activity and the risk of injury. I voluntarily allow my child to participate in this program.

I agree that Coach Faas, Next Level Soccer Training, and any associated staff or locations are not responsible or liable for any injuries, accidents, losses, or damages that may occur during participation.

I confirm that my child is physically able to participate in soccer activities and I assume all risks associated with participation.

I release Coach Faas and Next Level Soccer Training from any and all liability now and in the future.

Schedule & Cancellation Policy

I understand that Next Level Soccer Training sessions may be canceled due to bad weather, field conditions, or if Coach Faas is sick or unable to conduct the session. I understand that there are no make-up days and no refunds for missed or canceled sessions for any reason.
